Referrals can be made by many professionals: Ask your GP, Social Prescriber, Mental Health Practitioner, Housing Officers, Citizens Advise Bureau, and other health and social care organisations
Self-referral
We are not currently offering a self-referral pathway, but we have plans to in the next 12 months.
