What kind of information do you keep?
- Contact information for you (or your parent / carer) and other people involved in your care.
- Information about your background.
- Information about any assessments and questionnaires about you.
- Information about our plans for your future care.
- Details of the care we give you and correspondence related to your care.
Keeping your personal details up to date
It is important that you tell us as soon as you can if you change your details, telephone numbers or address. We will continue to use the address and telephone numbers you have given us until you tell us they have changed.
How do you store information about my care?
We keep information about your care on a dedicated specialist computer system within the service. Sometimes we keep paper records. They are stored securely in accordance with Trust policy requirements.
How is this information used?
We use information to check the quality of your care we are providing. We also use it to plan your treatment. We use it to improve the quality of the services that we offer and plan future services.
Can I see my records?
Yes, we are happy to provide you with a copy of your records. You will need to write to us to request these.
When would the service share my records?
You have control over who else is involved in your care. We observe strict NHS standards of confidentiality. The only time we will inform others without your permission is:
- If there is a potential risk of harm to you or others, including children. Or if there has or may have been a crime committed we are oblige to share this information.
- If you tell us that you are currently or have in the past been abused or hurt by another person we may have to share this information to protect others.
- We provide relevant information to your GP and other health and social care professionals where appropriate. Please inform us if you would prefer that we did not share information with other professionals. We will share information if there are concerns about safety as highlighted above. We will try to contact you first if this happens and do our best to help you.
